Description of Responsibilities

Responsible for assisting the patient with assigned tasks according to the service plan.

Responsibilities and Job Requirements

  • Assistance with bathing, toileting, grooming, shaving, dental care, dressing and eating
  • Basic first aid and CPR
  • Caring for clients with special conditions and needs so long as the services are within the scope of the tasks authorized to be performed by demonstration
  • Home Management
  • Home safety and sanitation
  • Infection control in the Home
  • Medically related activities to include: taking vital signs
  • Skin care and Nail care (excludes cutting of nails)
  • Ambulation and transfer, including positioning
  • I&O monitoring
  • Range of motion exercises
  • Assist with taking weights
  • Catheter and bowel care
  • Proper Nutrition: prepares meals and assists with dietary and nutritional needs
  • General light housekeeping, laundry, and shopping specific to patient
  • Observe and report significant changes in patient’s condition to the RN supervisor
  • Completes all documentation upon completion of shift
  • Processes time according to policy
  • Turns in documentation on a weekly basis
  • Accompanying the patient to medical providers
  • Maintains a mature and responsible relationship with staff, patients, and families
  • Assists with wheelchairs and other medical equipment and devices (non-life support)
  • Is dependable and on time for assignments
  • Is flexible and able to adapt to new situations

Minimum Qualifications

  • High School graduate preferred or equivalent of education and training
  • Current CPR Certification
  • Successful completion of a nurse aide training and competency evaluation program pursuant to the requirements of 42 CFR part 483, Subpart D OR successful completion of a competency examination for nurse aides recognized by the state OR successful completion of a health care or personal care credentialing program recognized and approved by the state OR successful completion or progress in the completion of a 40 hour training program provided by a private home care provider which addresses at least the areas specified in the PHCP rules.

  • Ability to keep simple records
  • Pediatric and Adult experience as appropriate for the patient
  • Never have been shown by credible evidence (court or jury, department investigation or other reliable evidence) to have abused, neglected, sexually assaulted, exploited, or deprived any person or to have subjected any person to serious injury as a result of intentional or grossly negligent misconduct as evidenced by an oral or written statement to this effect obtained at the time of application.
  • Participate in the orientation and training required by PHCP rules
  • Not have made any material false statements concerning qualificaitons requirements either to the department or the provider

Physical Requirements

  • Frequent lifting and repositioning of patients
  • Repeated or prolonged standing, bending, kneeling, twisting and occasional climbing of stairs
  • Acute sense of sight and hearing (corrected or uncorrected) and strong sense of touch, smell and taste
  • Agility and strength sufficient to handle patients and equipment without assistance
  • Must be able to appropriately respond physically and psychologically to emergency situations in the home or during transport
  • Must be able to function in a wide variety of environments which may involve exposure to allergens and other health conditions
